• If you have already been approved for Short-Term Disability (STD) coverage, it is not necessary to file a separate LTD claim. Principal administers both the STD and LTD programs and will transition your claim information for review.
  
  
• If you are not enrolled in the STD program or were not approved for STD benefits, it is your responsibility to contact Principal at 1-888-363-8208 to initiate an LTD claim.

• After you have contacted Principal, complete and mail the Long-Term Disability claim form to:

Des Moines , IA 50392-0002

• Long-Term Disability payments will be made to you by Principal, the administrator of the plan. All questions regarding LTD payments should be directed to The Principal.
