| How to Apply for Short-Term Disability Benefits-Non-Stores |
- Notify your Supervisor of your need for a medical leave of absence.
- Contact the HR Benefits Department immediately for the necessary Leave of Absence paperwork.
- Follow the checklist provided including the completion of the Leave of Absence Worksheet. This form must be approved by both your Supervisor and Manager and returned to the HR Benefits Department before your leave can be processed.
- Principal will contact you regarding the approval or denial of your STD claim.
- For approved requests, Principal will begin making payments after the completion of the 14 calendar day elimination period following the date of disability.
- Disability payments are paid by Principal, the administrator of the program.
- Retroactive payments will be made if the disability is approved after the Associate's earliest eligibility date for payments.
