How to Provide Evidence of Good Health
  • Select the plan and the desired coverage level on the enrollment panel of My Info.

  • You will see a warning box reading that 'proof of insurability is required'.

  • The My Info elections will be processed at the Corporate Office, the HR Benefits Department will be notified. They will send you the required evidence of good health paperwork for completion

  • Once the paperwork has been completed and signatures obtained, you must send the paperwork directly to Principal

  • The paperwork will be processed by Principal and they will respond to Payless with their determination

  • Principal will notify the HR Benefits Department regarding your approval or denial

  • Your new approved coverage will be effective with the beginning of the pay period following your notification

If applicable, Associate elections up to one increment will be allowed to update and any additional increments will not be applied until approval from Principal is received and the Associate has been notified.

  • For Associates ages 70 and over, evidence of good health is required for coverage greater than $10,000.
