If you do not have an ID card and need to use your benefit:

• Tell your provider that you participate in a Cigna plan
• Give your provider your Social Security Number
• Give your provider the Payless medical account number, Account #3180304
• Your provider can contact Cigna to verify your coverage status as of that date.

Once you receive your plan ID card, you should carry it with you and have your provider make a copy of it when you receive services.
