HMSA offers two plan choices to Associates in Hawaii. One is an HMO plan and the other is the CompMed plan.

Eligibility:

	When to Enroll		Coverage Effective
HMSA Full-time - with employer contributions	Within first 31 days of hire		First pay period following one month of service
HMSA Part-time *	Within first 31 days of hire		First pay period following one month of service

HMSA premiums are deducted in accordance with Hawaii state law.

*Associate pays full premium for dependent coverage.
