United 350 Medical Plan
  • The United 350 Plan is administered by UnitedHealthcare.

  • The United 350 plan has an annual $350 individual deductible and a maximum family deductible of $1,050.

  • In-Network Care

    • The United 350 plan provides a higher level of benefits when you use providers that are part of UnitedHealthcare's ChoicePlus network.

    • In addition to a higher level of benefits, you also get to take advantage of discounts offered by the UnitedHealthcare network of providers.

    • To find a network provider logon to the www.uhc.com or contact UnitedHealthcare.

    • Primary Care physician office visits are paid in full after a $20 co-pay and specialist visits are paid in full after a $35 co-pay

    • In-network deductibles are $350 individual and a maximum family deductible of $1,050

    • Out-of-pocket maximums are $2,500 individual and $5,000 family.

    • The plan pays 80% of your expenses after you meet the annual deductible and applicable co-pays. However, some services may not be covered or coverage may be limited. If you want to know how or whether a specific service is covered, contact UnitedHealthcare.

  • Out-of-Network Care

    • The United 350 plan also allows you to obtain care from providers that are not part of the UnitedHealthcare ChoicePlus network.

    • The out-of-network deductible is $1,000 individual and a maximum family deductible of $3,000.

    • The out-of-network out-of-pocket maximums are $5,000 individual and $10,000 family.
